Clinical Tips

General

  • Utilize tooth-coloured composite or acrylic for closing access holes in teeth.
  • For access holes in the acrylic base, opt for pink composite or acrylic.

Recommended restorative sequence for screw retained hybrid dentures 

1. Impression taking 

  • Take an implant level impression including vestibules and the palate for maxillary restorations 
  • Remove healing abutments or appliance
  • Seat the impression copings or scan bodies, a radiograph may be taken to confirm complete seating. 
  • Scan or take a PVS impression, loosen the screws and remove the impression copings/scan bodies 
  • Replace healing abutments

2. Jaw relation and shade selection 

  • Remove healing abutments or appliance
  • Seat bite block and tighten screws 
  • Take jaw relation records using conventional denture techniques 
  • Remove screws and bite blocks, replace the healing abutments or appliance
  • Take impression of opposing dentition and current denture if present 
  • Select tooth and gingival shade as well as tooth shape

3. Try in 

  • Remove healing abutments or appliance 
  • Seat the denture base with teeth in wax 
  • Tighten temporary screws 
  • Assess the VDO, CR, midlines, phonetics, shade, tooth appearance and occlusion
  • Unscrew the temporary screws and remove the set up 

4. Verification jig and final impression

  • Seat each section of the jig on to the implant or multiunit abutment and tighten guide pin. The sections must not be in contact and so trim with a disk if required 
  • Lute the sections together with a suitable material, the material should be allowed to flow through the gaps and it should be cured completely. 
  • Ensure adequate fit of the custom tray with no contact with the jig or cylinders. 
  • Take PVS impression making sure that impression material is injected under the implant jig. Once material has set remove the guide pins and then the impression (with the picked up verification jig) 
  • Replace the healing abutments or appliance 

5. Framework try in 

  • Remove healing abutments or appliance 
  • Seat the framework with the final denture set up in place 
  • Verify framework fit
  • Tighten one screw and verify a passive fit on all the implants or multiunit abutments. Repeat this process for the remaining implants or multiunit abutments 
  • Verify the VDO, CR, midlines, phonetics, shade, tooth appearance and occlusion
  • Remove the framework and the final set up 
  • Replace healing abutments or appliance 
  • Return to Boris Dental Labs for final processing 

6. Delivery 

  • Remove the healing abutments or appliance 
  • Seat the denture on the implants or multiunit abutments 
  • Starting from one side to the other tighten the screws to the appropriate torque recommended by the manufacturer
  • Confirm occlusion 
  • Make adjustments as required 
  • Place a small piece of cotton in screw access holes and fill with light cure composite or acrylic to prevent bacteria build up 

Maintenance 

  • A six month prophylaxis hygiene appointment should be arranged 
  • After twelve months the prosthesis should be removed for thorough cleaning and to assess screws – if there are any signs of wear or damage these should be replaced
