Benefits

  • Compensatory Fit: In scenarios like large-span bridgework with cast-metal frameworks, cement-retained solutions can compensate for potential inaccuracies that might arise from casting or post-ceramic firing distortions.
  • Angulation Flexibility: The design of cement-retained restorations offers leeway in implant angulation. This means that the exact screw access position becomes less critical, as the abutment can adjust accordingly.
  • Natural Aesthetics: The absence of a screw access hole in cement-retained restorations allows for a more natural-looking crown or bridge, closely mimicking the appearance of natural teeth.
  • Versatility in Design: Cement-retained restorations offer flexibility in design, allowing for adjustments and modifications to achieve the desired shape, contour, and shade.
  • Reduced Component Complexity: With fewer components involved compared to screw-retained systems, there's a reduced risk of mechanical complications like screw loosening.
  • Optimal Load Distribution: The cement layer can act as a stress absorber, distributing occlusal forces more evenly and potentially reducing the risk of implant overload.
  • Broad Applicability: Suitable for both anterior and posterior restorations, cement-retained solutions can be used in a wide range of clinical scenarios.

Clinical Tips

Impression taking – Traditional

  • Remove the healing abutment and replace it swiftly with an open- or closed-tray impression coping
  • Ensure the impression coping is seated correctly via a radiograph.
  • Record the impression with light body PVS around the implant coping and fill the impression try with medium/heavy body.
  • Submit to the laboratory.

Impression taking – Digital

  • Remove the healing abutment and promptly attach the scan body specific to the implant system
  •  Ensure the scan body is fully seated. Some clinicians opt to take a radiograph for verification.
  • Using your intraoral scanner, initiate the scanning process. Begin with the quadrant containing the implant, ensuring the entire scan body is captured.
  • Continue scanning the opposing arch and then record the bite registration.
  • Review the digital impression for clarity and completeness. Ensure all pertinent areas are captured without distortion.
  • Export the digital impression in the appropriate format (often .STL) and submit to the laboratory

Restoration placement

  • Ensure the custom or stock abutment is fully seated into the implant, aligning with the anti-rotational features of the connection interface.
  • Place the provided titanium screw into the screw access hole and hand-tighten using the designated driver. It's essential to take a radiograph at this stage to verify the abutment's complete seating.
  • With the correct driver and a calibrated torque wrench, secure the abutment according to the implant manufacturer’s specified torque value.
  • Seal the screw access hole using materials like cotton, Teflon tape, or gutta-percha.
  • Proceed with the recommended cementation process to attach the final restoration to the abutment.

Cementation

Based on the clinician's judgment, select either a temporary or permanent cement for the procedure.

It's crucial to ensure that any excess cement is thoroughly cleaned from around the restoration margins. Failure to do so, especially with subgingival margins, can lead to peri-implant mucositis or even peri-implantitis.
